FOSTER HOME AGREEMENT FORM
Please read the following conditions carefully.
These conditions apply to Animal Aide companion animals.
- All animals belong to the Animal Aide Centre
and must be returned to the centre at
the end of the foster period. If you are interested in
adopting one of these animals, you must apply for
adoption through the normal procedures, keeping in mind
that Animal Aide has the final say on the adoption approval
or denial. If denied, the animal must be returned to
Animal Aide immediately.
- In
the event that an animal becomes ill during the foster period,
you must contact the Animal Aide Centre immediately.
An appointment will be set up as quickly as possible for the
animal at OUR veterinarian. Unless prearranged
(see important notes below), all animals MUST be seen
by Animal Aide's veterinarian. In an emergency situation,
the vets are on call 7 days a week, 24 hours a day.

If fostering in London or surrounding areas, something
else could be arranged, but must be approved by
Animal Aide first.
- An
Animal Aide representative may contact you to arrange a visit
during the foster period. You may be required to show the room/area
in which the foster animals are being housed.
It is important that the foster homes meet the requirements
of the animal(s) being fostered. Animal Aide has the right to
remove any Animal Aide animal from the foster home at any time.
- It is
the responsibility of the foster caregiver to phone the
Animal Aide Centre at (519) 633-3788 and report how the
foster animal is doing. This should be done (minimum) once
weekly and immediately for any emergencies.
Leave a message on the answering machine or ask the volunteer
to record the message on paper and place in the animal's file.
If an emergency occurs after hours, call one of the emergency
clinic numbers listed above. Remember - if you are not sure, call!
It is better to be safe than sorry.
DISCLAIMER: The foster caregiver understands that the Animal Aide
Centre or its representatives will not be held liable for any sickness
or treatment of the foster
parent's animals resulting from the foster animal, and/or damage to
the foster home or for the injury to persons residing or visiting therein
during the period of fostering outlined in the agreement above.
